1. ACCEPTANCE OF TERMS & AUTHORIZATION

By accessing and using this Dehydration Management Calculator ("Tool"), you accept and agree to be bound by these Terms and Conditions. If you do not agree to abide by the above, please do not use this service.

1.1 Access Authorization - UPMC QI Project Only

This Tool is exclusively restricted to UPMC personnel actively participating in this Quality Improvement (QI) Project on dehydration management. Access requires a valid 4-digit access code. By entering the correct access code, you confirm that you are:

• An authorized UPMC healthcare professional or team member employed by or formally affiliated with this UPMC QI Project at UPMC Children's Hospital of Pittsburgh
• Using this Tool solely for authorized QI project purposes and clinical education
• Aware that this Tool is used across multiple clinical settings including the Emergency Department and hospital floors
• Subject to institutional UPMC privacy and security policies
• Authorized to access restricted medical educational materials and QI project resources

1.2 Unauthorized Access Prohibited

Unauthorized access to this Tool is strictly prohibited. Only UPMC Children's Hospital of Pittsburgh team members are permitted to share or have access to the access code. Users who access this Tool without proper UPMC QI Project authorization are subject to:

• Immediate termination of access
• UPMC institutional disciplinary action
• Legal prosecution under applicable laws
• Civil liability for damages

2. DISCLAIMER OF LIABILITY

2.1 Medical Disclaimer

This Tool is NOT a medical device, diagnostic tool, or clinical decision support system. It is intended solely for educational and informational purposes. The information generated by this Tool:

• Does NOT constitute medical advice, diagnosis, or treatment
• Should NOT be used as a substitute for professional medical advice
• Must NOT be used for clinical decision-making without validation by qualified healthcare professionals

2.2 No Liability for Outcomes

To the maximum extent permitted by law, the creators, developers, and distributors of this Tool shall NOT be liable for any:

• Any adverse outcomes, medical errors, or patient harm resulting from your use of this Tool are your sole responsibility
• The creators, developers, and distributors accept no liability for any consequences of your use of this Tool

3. APPROPRIATE USE ONLY FOR UPMC QI PROJECT TEAM MEMBERS

This Tool is intended EXCLUSIVELY for authorized UPMC QI Project team members working on dehydration management improvement initiatives. This includes use in the Emergency Department and on hospital stations/wards as part of the QI project implementation. Access is restricted via 4-digit access code. Authorized users must:

• Be formally part of the UPMC QI Project on dehydration management
• Have appropriate medical training and credentials for their role
• Be employed by or formally affiliated with UPMC
• Use this Tool only in compliance with applicable medical standards, UPMC policies, and regulations
• Never rely solely on this Tool for patient management decisions
• Understand that clinical judgment and professional expertise supersede any output from this Tool
• Maintain strict confidentiality of the access code
• Not share access credentials with unauthorized persons outside the UPMC QI Project team
• Use this Tool consistently across all QI project settings (Emergency Department and hospital stations/wards)

4. NO MEDICAL ADVICE

This Tool does not provide medical advice. Any information generated should be independently verified against current medical literature, clinical guidelines, and professional expertise. All clinical decisions must be made by qualified healthcare providers with appropriate patient evaluation.

5. ACCURACY DISCLAIMER

While we strive for accuracy, the creators cannot guarantee that:

• All information is accurate, complete, or current
• The Tool will function without errors or interruptions
• The Tool is free from viruses or other harmful code
• Any algorithms or calculations are clinically validated for your specific use case

6. NO PROFESSIONAL MEDICAL RELATIONSHIP

Use of this Tool does NOT create a doctor-patient relationship or establish any professional relationship between you and the creators. No healthcare provider associated with this Tool is providing medical services to you.

7. LIMITATION OF LIABILITY

TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT SHALL THE CREATORS, DEVELOPERS, DISTRIBUTORS, OR ANY AFFILIATED PARTIES BE LIABLE FOR DAMAGES OF ANY KIND, HOWEVER CAUSED, ARISING OUT OF OR RELATING TO THIS TOOL OR ANY INFORMATION CONTAINED THEREIN, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

8. REGULATORY COMPLIANCE

This Tool is provided without FDA approval or clearance. It is not intended to be used as a medical device as defined under 21 U.S.C. § 321(h) and 21 CFR § 860.3. Users understand and accept that this Tool may not comply with FDA regulations or other regulatory requirements applicable to medical devices or clinical decision support systems.

9. INDEMNIFICATION

You agree to indemnify, defend, and hold harmless the creators, developers, and distributors from any and all claims, damages, losses, or expenses arising from:

• Your use or misuse of this Tool
• Reliance on information provided by this Tool
• Any medical adverse events or patient harm
• Violation of these Terms and Conditions
• Violation of any applicable law or regulation

10. NO WARRANTY

THE TOOL IS PROVIDED ON AN "AS-IS" AND "AS-AVAILABLE" BASIS. TO THE MAXIMUM EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS, IMPLIED, STATUTORY, OR OTHERWISE, INCLUDING ANY IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, OR NON-INFRINGEMENT.

11. MODIFICATION OF TERMS

We reserve the right to modify these Terms and Conditions at any time. Your continued use of the Tool following the posting of revised Terms means that you accept and agree to the changes.

12. GOVERNING LAW

These Terms and Conditions are governed by and construed in accordance with the laws of the United States, and you irrevocably submit to the exclusive jurisdiction of the courts in that location.

13. SEVERABILITY

If any provision of these Terms and Conditions is found to be unlawful, void, or unenforceable, the remaining provisions shall continue in full force and effect.

14. ENTIRE AGREEMENT

These Terms and Conditions constitute the entire agreement between you and the creators regarding the use of this Tool and supersede all prior or contemporaneous agreements or understandings.
